Assisting with paperwork or prescription information 6. Enhancing coordination of care for primary care and specialty team members 7. Enhancing adherence, compliance, and understanding for care plan and treatment 8. Discussion for holistic services and local recommendations 9. Providing Home Health Care- post-discharge, post-surgery, post-op, post-stroke care, and education in the home setting 10. Compilation of questionnaires to ask healthcare professionals
